Teachers please download & mail in the following forms:

• Group Registration - Please mail this in by April 20th. Please remember to list your departure time and the number of students in each grade so that we plan the schedule for the day!
  
• Youth Registration - Each student will need to complete & return this form
• Teacher / Adult Leader Registration  - each adult must complete and return this registration form and a Voluntary Disclosure Statement**
  

* Voluntary Disclosure Form - For liability purposes and in best interest for the students, all adults working with student at camp are required to complete and turn in a Voluntary Disclosure form to the Outdoor Education Manager or lead staff upon arrival. School who've already completed background checks on their adult volunteers may bring a signed letter on letter head from the school administrator with the volunteer names and dates they performed a background check.
